Now we are at the heart of the fix.  These items appear to be copies of the old and they fit.  All is well, the repair items were shipped quickly and in my hands much sooner than anticipated and all this for a reasonable price.

5 of 5 LiftMaster® 41C4220A Gear & Sprocket repair kit November 25, 2016

A very complete kit, with clear instructions. Easy to install, except for the worm gear, which is virtually impossible to remove from the motor axis, but it was undamaged, so I merely installed the pinon gear; the original had been destroyed. I wish I had bought only that, instead of the complete kit.
Star Door Parts shipped the   LiftMaster® 41C4220A Gear & Sprocket Assembly kit quickly and cheaply, so I am very satisfied with their service.
This is the second door opener I have repaired this year with these kits. They work and are very cost effective. I am very satisfied with their price and quality.
I would buy again (but I hope my openers will last another 15 years before needing repair).
